These terms cover the website. The care we provide is governed separately by the consent forms and practice policies you review and sign as a patient.
This website is not medical advice
Everything on this site is general education. It is not medical advice, a diagnosis, or a treatment plan, and it is not a substitute for care from a licensed clinician who knows your history.
Reading this site, sending us a message, or booking an appointment does not create a clinician and patient relationship. That relationship begins when you are evaluated by one of our clinicians and accepted for care.
Never delay or disregard medical advice because of something you read here. If you think you are having an emergency, call 911 or go to the nearest emergency department. This site is not monitored for emergencies.
Who we can treat
Care is provided by telehealth and only to patients who are physically located in a state where our clinicians are licensed. You will be asked to confirm where you are located.
You must be an adult and able to give your own consent to treatment, unless we have agreed otherwise in writing. You agree that the information you give us is accurate and complete, and that you will keep it current. We rely on it to care for you safely.
Booking, payment, and cancellations
Booking a visit reserves time with a clinician. It is not a purchase of a medication or a guarantee of any particular treatment.
We are a self-pay practice. Prices shown on this site are current at the time of publication and may change. The price you are quoted at your visit is the one that applies.
If you need to cancel or reschedule, please tell us as early as you can so the time can go to another patient. Any cancellation terms that apply to you are given to you in writing before your visit.
Calls, texts, and email
If you give us your phone number or email address, you agree that we may contact you about your care, your appointments, and your account. Message and data rates may apply. You can opt out of non-clinical messages at any time by telling us, or by replying STOP to a text.
Ordinary email and text are convenient but not fully secure. Please do not send us sensitive health information through them, and never use them for anything urgent.
Your account and your conduct
If the site gives you a login or a patient portal, you are responsible for keeping your credentials private and for what happens under your account. Tell us right away if you think someone else has access.
Do not attempt to disrupt the site, gain access to areas you are not authorized to reach, scrape it, or use it to break the law or to harm anyone.
Content and ownership
The text, images, layout, and design on this site belong to M&K Primary Care & Wellness or to the people who licensed them to us. You may read and print pages for your own personal use. You may not republish, sell, or use them commercially without our written permission.
Any review, comment, or photograph you send us may be used only with your written permission, and you can withdraw that permission at any time.
Links to other sites
This site may link to services we do not run, such as a booking system or a payment processor. We do not control those sites and are not responsible for their content or their privacy practices. Their terms apply when you use them.

These terms are governed by the laws of the State of California, without regard to conflict of law rules. Any dispute about this website will be brought in the courts of California.

We may update these terms. The date below tells you when they last changed. Continuing to use the site after a change means you accept the updated terms.
